In order to be eligible for the Coronal Polishing course you must provide the following information along with your application and payment:

• A copy of current Ohio Dental Assistant Certification or DANB Certification
• A copy of your current CPR certification
• Proof of  Hepatitis immunization
• Completed Medical History form (to be provided by Sinclair after we have received your completed application and proof of the above certifications)
