Person Proxy and Student Record Release

Sinclair Community College offers two ways for a student to give access of their student record.  By granting access either means, you are giving Sinclair College permission to release information specified by you the student.  The consent submitted must specify what information to be released.

 

Person Proxy

Allows a student to authorize the release of Student information via online only.  This process allows one or more named person(s) online access for the chosen areas the student gives access.   The authorized person will be able to log in to my.sinclair.edu as a proxy of the student to see the authorized areas.

The following areas are available to give access and listed are the items included:

  • Academics: Grades
  • Financial Aid: Offer Letter, Financial Aid Home, My Awards, FA Required Documents, Satisfactory Academic Progress, College Financing Plan
  • Student Finances: Account Activity, Account Summary, Make a Payment

Person Proxy can be given by the student only by following the below:

  1. Log into my.sinclair.edu
  2. Click on Colleague Self Service (by a Tile, drop down, or a search)
  3. Click User Options (on left or click user name at top right corner)
  4. Click View/Add Proxy Access
  5. Use the drop down to Select Add a Proxy
  6. Enter required information
  7. Select Areas you want this specific person to have access

To change or remove access, the student would follow the same directions except click the pencil to change or revoke all access

If the proxy believes they do not have the correct access, only the student can control this by following the directions above.

STUDENT RECORD RELEASE

Allows a student to authorize the release of their Student Information in person or over the phone.   This process allows one or more named person(s) to be given access by using a designated PIN decided on by the student to discuss the chosen areas to which the student has granted access for the authorized user.
 

The following areas are available to give access and listed are the items included:

  • Advising: Academic Interactions, Academic Progress
  • Cashier/Bursar: Includes but not limited to Debt and collection info, Financial Holds, Other payment/funding info,, Payment Plan, Student Account Balances
  • Faculty: Assignments, Attendance, Discuss academic process, Grades Earned
  • Financial Aid:  includes all general financial aid information
  • Registration & Student Records: Current enrollment, Date of application and program selected, Dates of enrollment, enrollment status, Mailing address information, Posted Grades (this does not include why a grade was given, please see Faculty), Tuition Residency Rate, Terms attended

Student Release can be given by the student only by following the below:

  1. Log into my.sinclair.edu
  2. Click on Colleague Self Service (by a Tile, drop down, or a search)
  3. Click User Options (on left or click user name at top right corner)
  4. Select Student Records Release
  5. Click Add Person/Relationship
  6. Enter required information
  7. Select Areas you want this specific person to have access

To change or remove access, you would follow the same steps of logging in above to access

If the person given access does not remember the pin number given or does not believe they have the correct access, only the student can give and/or edit this information.   

I understand that the student records information includes information which is classified as private under the federal Family Education Rights and Privacy Act, 20 U.S.C. 1232(g).  I understand that by giving access I am authorizing Sinclair Community College to share to the persons given access, which would otherwise be private and not accessible to them.  I understand that without my informed consent, Sinclair Community College could not release the information described above because it is classified as private.

I understand further that (1) I have the right not to consent to the release of my education records; (2) I have the right to receive a copy of such records upon request; (3) and that this consent shall remain in effect until revoked by me, in writing to the office (s) it could have been provided.   I acknowledge that any such revocation shall not affect disclosures previously made by Sinclair Community College prior to the receipt of any such written revocation.  I acknowledge that the release must be revoked by me in my.sinclair.edu, Colleague Self-service.

This information is released subject to the confidentiality provisions of FERPA, which prohibits any further disclosure of this information without the specific consent of the person to whom it pertains.
